Acute right MCA territory infarction

Clinical presentation

65 year old gentleman presented with acute onset left sided weakness since waking up in the morning.

Findings

Scattered areas of hypoattenuation are seen involving the subcortical and deep white matter of right frontal lobe, corona radiata and external capsule. Diffusion weight MR images confirm restriction in keeping with acute cerebral infarction. No evident hemorrhagic transformation.

Discussion

Classic case of cerebral arterial territorial infarction Рright MCA. Additional negative reporting for any hemorrhagic transformation, dense blood vessels supplying the affected region and also to look for any form of crossed cerebellar diaschisis.
